Plan Review & Inspection Application
East Grand Fire Protection District No. 4 requires permit fees for plan reviews and inspections. Reference the fee chart on the Plan Review Permit Fees page for more details. For your convenience, this is a fillable form using Adobe Reader which can be saved on your computer or tablet. There is a “Submit Form” button that is easy to use if you have your email setup in an application like Outlook or Mail, or if you are familiar with your incoming and outgoing mail servers. Otherwise, just save the completed form to your computer and submit it as a normal email attachment to egfd@eastgrandfire.com. * Please note, the form must be opened in Adobe Reader in order to use the fillable form feature. It can be opened in a browser window (Firefox, Chrome, Safari, etc) to print the form, but you have to download the form and open it in Adobe Reader in order for the fillable form feature to work.

Fireworks, Pyrotechnic and Flame Effect Productions Permit & Requirements
An approval and permit is required for fireworks, proximate audience displays, flame effects and pyrotechnic special effects in motion picture, television, theatrical, and group entertainment productions.
